Telemedicine Digital Registration Instructions

Before Your Scheduled Virtual Appointment 

Complete the Digital Registration

Our digital registration is simple and should take approximately 10 minutes to complete. The confidential registration forms should be completed in its entirety before submitting and confirming your upcoming appointment. Mandatory fields and e-signatures are indicated.

Please be aware that your registration will not be considered complete unless all required forms and documents have been submitted. 

Step 1

Log in

Log in to your Patient Health Portal

Your upcoming appointment appears on your home page and on the Appointments page.

Click "Check in now" 

Step 2

Confirm Details

1. Confirm the date and time of your scheduled appointment

2. Confirm or update your contact and personal information.
​

3.  Select Yes to receive text alerts from our office about your appointment.

4. Click Save and Continue.

Step 3

Digital Registration

  • Update your Health History and eSign all documents and consent forms.
  • Submit your current insurance card, government identification and payment if required. 

You are now ready for your appointment! 

You will receive a text and/or email with your virtual appointment link on the day of your scheduled appointment.
